Project Description

Context and Objective of Project
Rodrigues is an autonomous island holding approximately 41667 inhabitants with a young aging population according to Digest of Statistics on Rodrigues 2014 published July 2015. Presently, Rodrigues holds one hospital - Queen Elizabeth Hospital at Crève Coeur, two Health Centres at Mont Lubin and La Ferme, and 14 Community Health Centres around the island which deliver free health care services.
Doctors are posted in hospital and Health Centres on a continuous and rotational basis to ensure a permanent health care service. Actually, there are four doctors posted in both Queen Elisabeth hospital and Mont Lubin Health Centre and three in La Ferme Health Centre. Each Community Health Centres receives on average once a week the visit of a doctor for their routine check-up and for consultation.

Actually in Rodrigues, doctors deliver only generalist and specialist services. As at date, there are 19 generalist doctors who ensure a daily and continuous health care on a roster basis where 8 (6 permanent and 2 on a month to month basis) of them are under the Rodriguan establishment, and the remaining 11 fall under the Mauritian establishment. The recruitment contract agreement of Mauritian generalists is being signed on a minimum of 6 months and up to a maximum of 3 years. As a result, to be more patient-centred, a concept of Community/family doctor was announced in the last Chief Commissioner’s budget speech in October 2015 in view to increase both the doctor-patient relationship as well as the effectiveness of the medical sector.

The role of the Community/Family doctor will be to provide a quality and up to date health care delivery system based on the concept of preventive medicine. The concept will also:

  • Reduce time taken on finding patient’s files and patient’s waiting time;
  • Provide continuing, comprehensive health care for the individual and family while centring this process on the patient-doctor relationship;
  • Give professional advice in promoting health, modifying health risk factors and preventing diseases (sensitizing);
  • Provide support to patient in progressive or complex long-term health conditions which require input from nursing and allied health professionals (Chronic);
  • Help to reduce disease complications and improve health outcomes;
  • Minimise service duplication through better co-ordination and helps patients receive the optimal care based on their needs.

The Commission for Health and Sports has decided to implement the Community/Family doctor’s concept in the 14 Community Health Centres of the island. The RRA proposes to assign 25 doctors for this purpose, among which 5 will exceptionally be allocated for the care of elderly person and people with disabilities. The remaining 20 doctors will be in charge of patients locating the vicinity of the 14 Community Health Centres as well as the delivery of preventive care campaign in schools and shelters.

The purpose of this consultancy is to provide a clear and detailed document on how to effectively implement the Community/Family doctor’s concept in the Rodriguan community. The Consultant will work in close collaboration with the Commission for Health and Sports to understand how the actual health care system works in Rodrigues and for easy access to needed information. The expert will also work closely with the Commission for Social Security for facility access to relevant information related to people with disabilities and elderly person. The main responsibility of the expert will be to provide a document on how to efficiently and effectively implement this concept while concentrating on the patient-centred approach which is the main objective of this project. Furthermore, he/she will also have to deliver proposals on the different upgrading works to be made on the refitting of the existing building so as to accommodate the implementation of this project.

Duties and Responsibilities

Scope of Work

The consultant is expected to play an important role in defining and understanding the living environment of Rodrigues and its population density for the implementation of the concept. He/she will have to define the surrounding population that covers each Community Health Centre and based on it he/she will have to suggest the personnel required for the setting up of the concept. A site visit survey will also be needed for recommendation of required upgrading works and the refitting of actual Community Health Centres. The expert will have to work on the implementation of the Community/Family doctor in Community Health Centres by:

  • Carrying out a detailed analysis and survey on the number of people that surround each Community Health Centre in collaboration with the RRA;
  • Suggesting the number of personnel required in each Community Health Centres by taking into consideration its vicinity population;
  • Based on the number of elderly person and people with disabilities, the expert will have to set up a detailed planning on how effectively these 5 assigned doctors would implement the concept;
  • A detailed work plan of all the personnel working in the Community Health Centres by taking into consideration opening/closing hours, working days and days where doctors will have to carry sensitisation campaign in schools and shelters (should suggest which aging student to segment);
  • Developing a detailed document on the roles and responsibilities of each personnel working in the Community Health Centres;
  • Participating in one day working session for the inception and validation of the project;
  • Providing strategies on how this concept could be applied in regions of hospitals and Health Centres;
  • Undertaking surveys on existing infrastructure and equipment thus providing a detailed plan on how to upgrade the Community Health Centres.

Expected outputs and Deliverables

The key products to be delivered are as follows:

  • A location plan of all the Community Health Centres of the island with its surrounding population;
  • A detailed implementation strategy of the Community/Family doctor’s concept for both the Community Health Centres and elderly person/people with disabilities;
  • Scope of work of each personnel working under the Community/Family doctor’s concept;
  • A detailed plan for upgrading works that need to be carried out in the Community Health Centres;
  • A model Community Health Centre comprising of its personnel for the implementation of the concept.

The Consultant shall be remunerated in accordance with the following time schedule and deliverables:

Institutional Arrangements

While the consultant will be contracted by the UNDP in collaboration the Rodrigues Regional Assembly, the participating Commissions will have a key role to play for the implementation of the concept.

The institutional context of the project covers the mandates of the Rodrigues Regional Assembly. From the government viewpoint, the project is headed by the Commission for Health and Sports which is responsible for the medical welfare of the Rodriguan population.
The Consultant shall perform a liaison role between the Rodrigues Regional Assembly (mostly the Commission for Health and Sports) and the UNDP.

The Consultant shall work under the supervision of the Steering Committee (SC) and shall report to it for the achievement of its duties. The SC will provide high-level policy guidance and orientation to the project and will be chaired by the Island Chief Executive. The following representatives will form part of the Steering Committee:

  • Acting Hospital director;
  • Acting Departmental Head of the Commission for Health and Sports;
  • Departmental Head Commission for Social Security;
  • Male and female Nursing Administrator;
  • Senior Medical Officer;
  • Acting Senior Analyst;
  • Representative of UNDP.

Duration of work

This assignment is for duration of 7 days, starting no later than 13th June 2016 and ending 13th August 2016. It is envisioned that this assignment will require two (2) visits to Rodrigues (for a total of 5 working days) whereby during the second visit the consultant would hold a workshop will all stakeholders to present the final report.

Required Skills and Experience

Qualifications of the Successful Individual Contractor:

Education

  • Minimum Master’s Degree or equivalent in Health care planning, Project management (Community hospital) or any related field.

Experience:

  • Minimum of 8 years of demonstrative experience and knowledge in the field of Health Care Planning (infrastructure as well) and project management;
  • Strong expertise in related health planning projects;
  • Proven experience in introducing the community/family doctor concept in other countrie;
  • Proven experience in the field of preventive medicine.

Language:

  • Excellent written and spoken English and French are required;
  • Knowledge of Creole will be an added advantage.

Scope of Price Proposal and Schedule of Payments

Price Proposal:

  • The financial offer should be quoted as a lump sum amount, ‘all-inclusive ‘. The term “All inclusive” implies that all costs (professional fees, travel costs, living allowances, communications, consumables, etc.) that could possibly be incurred by the Contractor are already factored into the final amounts submitted in the proposal.
  • The contract price is fixed regardless of changes in the cost components. Payments will be effected based on deliverables.

Payment Schedule:

  • 20%  upon submission of Inception report;
  • 30 % upon submission of Survey report and recommendations on site visit for upgrading and refitting of Community Health Centres (including location plan) and Draft document on the implementation of Community/Family doctor in Community Health Centres and for Elderly Person and people with disabilities (including scope of work for personnel);
  • 50% upon Submission of Final document.

Proposed Work Plan and Indicative Timeline:

  • It is planned that the consultancy starts no later than 13th June 2016 and ending 13th August 2016.

Recommended Presentation of Offer

  • Duly accomplished Letter of Confirmation of Interest and Availability using the template provided by UNDP;
  • Personal CV and P11 (both), indicating all past experience from similar projects, as well as the contact details (email and telephone number) of the Candidate and at least three (3) professional references. Candidates not submitting P11 will not be considered;
  • Brief description of why the individual considers him/herself as the most suitable for the assignment, and a methodology on how they will approach and complete the assignment;
  • Financial Proposal that indicates the all-inclusive fixed total contract price, supported by a breakdown of costs, as per template provided.  If an Offeror is employed by an organization/company/institution, and he/she expects his/her employer to charge a management fee in the process of releasing him/her to UNDP under Reimbursable Loan Agreement (RLA), the Offeror must indicate at this point, and ensure that all such costs are duly incorporated in the financial proposal submitted to UNDP.

Criteria for selection of best offer

The award of the contract will be made to the Individual Consultant whose offer has been evaluated using the “Combined Scoring Method” and determined as:

  • Responsive/compliant/acceptable;
  • Having received the highest score out of a pre-determined set of weighted technical and financial criteria specified below (Technical Criteria weight (0.7), Financial Criteria weight (0.3).

Only candidates obtaining a minimum of 70 marks out of 100 would be considered for the Financial Evaluation.
