Background

Yemen is currently a critical and complex context. We need urgently an international Paramedic/Nurse who has to implement an annual work plan under the technical supervision of the UN Physician according to the activities described below

Duties and Responsibilities

  • Provide daily 24 hours emergency medical coverage for UN staff;
  • Accompany United Nations VIP and his /her protections team into an insecure environment and, if required, provide emergency medical intervention for casualties in the event of injury or illness of the VIP or his/her team.
  • Support the development of the emergency medicine capacity of medical facilities within the duty station and abroad, as required;
  • Conduct assessment of the medical facilities in Sanaa and other cities, as required.
  • Produce a report outlining recommendations on medical support for future UN missions to Yemen.
  • Develop emergency medical contingency plans and assist with UNCT/UNDSS’s planning for response to a mass casualty incident and medical evacuation.
  • Provide on-going emergency first aid training to the staff * Perform medical briefing and training for incoming staff.

Job Knowledge/Technical Skills

  • ValidPro-Hospital Trauma Life Support (PHTLS) and Advanced Cardiac Life Support (ACLS) certificates or equivalent (authorizing body of the certificates also must be mentioned);
  • Basic understanding of adult learning concepts and the development and delivery of emergency medical / training courses (including scenario based training);

Ability to wok unassisted in remote locations and perform the following interventions without supervision, including: 

  • Use of basic airway adjuncts such as OPA, NPA.
  • Provide airway suctioning. 
  • Perform emergency endotracheal intubation and manual (or mechanical) ventilation (IPPV and APPV).
  • Perform pharmacologically assisted intubation. 
  • Provide on-going monitoring using ECG, SpO2 and End tidal CO2 monitoring equipment.
  • Pharmacologically treat medical emergencies - hypoglycemia, anaphylaxis, continuous recurrent seizures, overdose of narcotic drugs, pulmonary edema, conscious / altered conscious asthma. 
  • Administer narcotic analgesia. 
  • Manage chest pain of cardiac nature using appropriate pharmacological intervention.
  • Provide appropriate intervention (including pharmacological intervention) for unstable symptomatic patients the following dysrhythmias: ventricular fibrillation, ventricular tachycardia, electromechanical dissociation, asystole, bradycardia, supra ventricular tachycardia, atrial fibrillation / flutter with rapid ventricular response, premature ventricular complexes, accelerated idioventricular rhythms, bizarre / unknown rhythms. 
  • Provide appropriate intervention for patient with inadequate perfusion associated with cardiac chest pain. 
  • Manage a casualty with hypo perfusion associated with hypovolemia / non-hypovolemia causes.
  • Provide intravenous and intra-osseous infusion. 
  • Emergency airway-needle crichothyrotomy.
  • Needle chest decompression.
  • Spinal immobilization using C-collar, head blocks, spinal board. 
  • Apply traction splint for lower limb closed fractures.
  • Use of Asherman chest dressing (or similar) for open pneumothorax injury.
  • Appropriately manage Traumatic Brain injury with evidence of raised intracranial pressure. 
  • Appropriately manage casualty with large surface area burns for protracted period.
  • Appropriately manage casualties with traumatic injuries including fractures, blast injury, crush injury, drowning / near drowning, open abdominal injuries, etc.
  • Monitoring and Progress Controls will provide the DO and MSD with a weekly and monthly report which will include the following information:
  • Training Activities Conducted: at least four sessions per month;
  • Upcoming Training Activities;
  • Update on Equipment and Supplies, maintenance program and disposable/waste management;
  • Update on oxygen tanks refilling and blood availability for transfusion;
  • Road Missions;
  • Medical Evacuations by Air, by Road, by location, by destination, … (according to theUN rules and regulation);
  • Patient Assessments Conducted;
  • Drafting and updating mass casualty and medical evacuation plans;
  • Incident report /adverse events;
  • Challenges difficulties and possibilities for continuous quality of care and patient safety.

Required Skills and Experience

Education:

  • Professionally Licensed Nurse / Paramedics (authorizing body must be cited);
  • Valid certification in Advanced Trauma Life support;
  • Advanced cardiac Life support OR Pre hospital trauma life support is desirable;
  • Valid certificate as a trainer for BLS, ACLS or PHTLS.

Experience:

  • At least 5 years of experience working in an emergency medical setting in an advanced life support (ALS) role;
  • Previous UN medical system/international medical experience is desirable.  

Language Requirements:

  • For the post advertised fluency in oral and written English is required;
  • Arabic is highly desirable.
