Psycho-social rehabilitation is one of the integral components of Victim Assistance component of the Mine Action in general, and the Tajikistan Mine Action Programme (TMAP), in particular.

Physical traumas created by mine/ERW explosions are among the most difficult injuries to treat and to live with. A person who has stepped on landmine usually suffers such severe disruption of the limb(s) that surgical amputation of one or both legs results even when a traumatic amputation is not immediate. If a person is injured while handling a mine/ERW, it often results in the loss of one or both hands, victims are frequently blinded or suffer traumatic injuries to the abdomen, chest and spinal column. While landmine survivors having Critical Incidence Stress (CIS) which is normal response to abnormal event (landmine explosion) are provided by the medical care and rehabilitation, but, as a rule, most survivors of landmine accidents have different immediate and delayed reactions to CIS, however, they usually do not receive first psychological aid (FPA) and psycho-social rehabilitation.

Since 2005 TMAP’s Victim Assistance Programme provided psycho-social rehabilitation support to more than 600 landmine/ERW victims and their families through different types of psycho-social support projects, namely, organizing of peer-to-peer support training for landmine survivors, summer rehabilitation camps for landmine survivors, capacity building training for local doctors and nurses on provision of the first psychological aid to persons with disabilities, including landmine survivors (2013-14). Projects were implemented in close cooperation with the key national partner – Ministry of Health and Social Protection of Population of RT (MHSPP), the Tajik National University (psychology department) and other VA partners: NGO “Harmony in the World”, “Tajikistan Campaign to Ban Landmines and Cluster Munitions” (TCBL&CM), Public Organization “Psychology Support Centre”, Red Crescent Society of Tajikistan and Society of Persons with Disabilities “Imkoniyat”.

External evaluation of the psycho-social support projects report 2016) revealed some difficulties and shortcomings in this component, namely, that the projects did not provide post-training support to participants, which diminished the projects’ effect. It is proved that educational projects should not end with one training session. Knowledge and skills need to be reinforced and improved after the completion of training, therefore abovementioned report recommended to organize Certification training in order to “prepare a certified team of trainers-cum-service providers, both from beneficiaries ("peer-to-peer" counseling) and medical/social workers”.  

Thus, at the first stage, it is proposed to change the approach and organize professional certification training for local doctors from mine-affected areas on provision of the psycho-social rehabilitation support in crisis situations. Training based on best advanced international experience and using innovative approach will contribute to integration of mental health issues into primary healthcare in mine-affected districts and thus will improve the effectiveness of the psycho-social component of Victim Assistance pillar and ensure sustainability of services.

Goals and objectives:

UNDP in Tajikistan seeks an International Consultant, who, under direct supervision of Disability Support Unit Advisor and in close consultation with STMAP Project Manager as well as in close cooperation with the Ministry of Health and Social Protection of Population (MHSPP) and other relevant counterparts will conduct professional certification training for medical staff from mine-affected areas on provision of psycho-social rehabilitation in crisis situations, including to landmine survivors, families of victims and other persons with disabilities based on new innovative techniques.

Duties and responsibilities:

The scope of work under this Terms of Reference focuses on preparation and organization of professional certification training for local doctors from mine-affected areas on provision of the first psychological aid and psycho-social assistance in crisis situations. In the framework of Victim Assistance pillar, International Consultant is expected to conduct Certification training on provision of the psycho-social support in crisis situations and thus will facilitate integration of mental health issues into primary healthcare. Specifically, International Consultant will perform the following tasks:

  • Identify training needs through pre-test questionnaire;
  • Propose innovative methodologies/ approaches to organize professional certification training for at least 20-25 medical staff (doctors) on provision of the first psychological aid working in the primary healthcare system in the mine-affected districts;
  • Develop work Plan and Programme for professional certification training based on advanced contemporary knowledge (home based);
  • Develop presentations for 5-days certification training and training materials, which should be send to DSU Advisor for translation at least one-two weeks before training;
  • Conduct professional certification training based on using of the new innovative techniques which  should include, but should not be limited to, the following topics – mental health, stress and traumatic stress, Post-Traumatic Stress Disorders (PTSD), suicide behavior, stress managements technics developed by UNDSS Critical Incidents Stress Management Unit, provision of the first psychological aid (FPA), creative art therapy, communication skills, cultural diversity, etc.
  • Conduct professional certification training for group of local doctors (20-25 participants); Training should consist from theoretical and practical parts (exercises, role plays etc.);
  • Develop relevant forms and templates to be utilized during planning and implementation process of the first psychological aid;
  • Conduct final examination of the trained participants and certification of successful participants;
  • Formulate practical and helpful recommendations for follow up and continuation capacity building for local staff;
  • Prepare the report on collected assessment data, an analysis of the findings, based on pre- and post-test has been done for the trainees and the training activities;
  • Provide recommendations to the project team on further implementation of psycho-social support work in the framework of STMAP;
  • Finalization and submission of the report in English.

Qualifications et expériences requises

Education:

  • Master degree in the field of psychology/psychiatry and/or any other related fields. 

Experience:

  • Minimum 5 years of professional expertise in mental health, including first psychological aid, stress management etc;
  • At least 5 years of work experience in humanitarian programming in the area of mental health and first psychological and psycho-social support;
  • At least 5 years of experience in conducting mental health and/or psychosocial support training;
  • Cross-cultural working experience in the field of community based psychosocial support is mandatory;
  • Demonstrated expertise in conducting participatory assessments and approaches;
  • Knowledge and experience implementing guidelines on MHPSS in Emergency Settings is mandatory.

Language:

  • Proficiency in English, excellent analytical and drafting skills; knowledge of Russian/Tajik/Persian language is an advantage.
